What is a Primary Care Network?
Primary Care Networks are a key part of the NHS Long Term Plan. Each network typically serves populations of 30,000–50,000 people and brings together a range of services to work collaboratively—GPs, clinical pharmacists, physiotherapists, mental health practitioners, social prescribers, and more—based on the specific needs of the local population.
- Improves coordination of care
- Keeps healthcare focused within communities
- Supports people to live healthier, more independent lives
